HUMAN REVIEW

What makes a human-review record actually useful?

Capture enough context to explain what was reviewed, by whom, when and against what evidence.

A status alone is not a decision trail

A green check mark tells a future reviewer almost nothing. A useful human-review record preserves the question that was considered, the human outcome and enough rationale to understand why the decision was made at that time.

Minimum useful decision context

  • The review question or operational prompt.
  • The system and client context the question applied to.
  • The human-selected outcome, including not-applicable where appropriate.
  • A short rationale in the operator's own words.
  • The evidence or source material the operator relied on.
  • Timestamp, responsible user and next review timing.

Make re-review normal

The value of the record is not only historical. When the deployed workflow, disclosure, vendor stack or operating context changes materially, the prior decision should remain visible while the current record is explicitly reviewed again.

That is more trustworthy than silently overwriting the old answer and pretending the decision was always current.

Scope note. This is operational workflow guidance, not legal advice or an automatic compliance conclusion.

If this operating problem is real for your team, a short controlled trial is more useful than another abstract checklist.
